The purpose of the form is to protect what you or you and another own.The form should be filled out and signed by the individual. I believe it can be in their own handwriting,not necessarily typed, although typing is more easily legible. A will is to protect assets, property, personal property, etc. This blank copy has room for three beneficiaries. These two copies are merely blank copies you would need to fill in one and register with your state County Courthouse,after being signed, witnessed, dated and notarized.There will be two copies, just in case a mistake is made, as almost all Counties will not accept a Document that has a scribbled out error within or on it.This is a form only for the State of Michigan. There are 5 pages.The first photo is a partial photo of Page 1. and the second photo is of the last page.I am not authorized to give legal advice.
